2008 Summer Program Forms
HEALTH FORMS
Maryland state law requires that each camper have a complete set
of health forms at each campsite where the camper is registered.
Completed health forms must be turned in prior to the first day of
the camp session. It is your responsibility to deliver all necessary
health forms to each campsite. Campers may not attend any camp
sessions without completed health forms on site. NO EXCEPTIONS.
Summer Day Camp Registration Forms
1. Mail-In Form - Summer Day Camp Registration & Payment
This form is required for mail-in and walk-in registrations only.

2. Day Camp Participant Profile Form
This form is required for all campers. Please note that camper will not be allowed to attend without completed form.

3. Day Camp Participant Medication Form
This form is required if participant requires any type of medication/ medical devices/special health procedures.

4. Day Camp Authorization Form
This form is required for every day camp participant at each site.

5. Maryland Department of Health and Mental Hygiene Immunization Certificate
This form is required at each site for any participant (ages 18 &s under) who did not attend a Maryland public or private school this year (i.e. home-schooled, out-of-state schools).
